#3  No succession planning and no substantial force on the ground.

No real assistance to the first government to root out the militias. The intel was there that the militias were mostly radical and the empty suit and the Russian agent in the State Department let it disintegrate.

#2  More importantly, Khaddafy openly threatened to open the floodgates and let the migrants through if he was not appeased. So they took him out to remove the threat... and the floodgates opened. Lose-lose. Unexpectedly.
