#1 Simply a quaint term of reference or approximation as opposed to an actual measurement; quite like 'winning by a nose', 'country mile', 'finger of scotch', 'ear full', or a 'butt load'. A 'baker's dozen' however, is an actual number.

#10 not exactly "America at its finest". I didn't notice anyone complaining about Subway sandwiches for the last 10-20 years. I doubt there's any regulation that says a sandwich must be accurate to one millimeter length.
