#1 I'm leaning towards the 'B' answer as the primary cause, with 'A' as Second and Third Order Effects.

(A) According to the Hamas-affiliated Al Resalah, the blast which killed the "martyrs" was caused by a fire that broke out in a house.

(B) Meanwhile, Israeli media reported that the blast had occurred during a training session on producing explosive devices.

#12 Actually it's not the red wire/green wire problem as to save money and effort they bought all pink wire since nobody wanted it and they got it dirt cheap.

So yeah, who needs to use different colored wires, I mean, it's easy to remember when assembling your kaboomite packages which wire goes were. Location location location.
