#4  It was intentionally left vague by SpaceX with no answers to questions. The booster did not have landing legs and no intention to land it. The booster did have it's big transonic steering paddles.
The booster was relit to everybody's surprise but no video or explaination. It did do all sorts of strange movements. It did crash into the sea.

Anything else is guesswork.

#2  The appearance of a second rocket breaking away from the primary was very curious and I have yet to hear an explanation of it.
