#1 two things jumped out at me:
upon landing the crew goes to full throttle in the event of a cable failiue so the cable didnt fail right away
and
the cable didnt whip around as is generally the case; that is probably why no deaths
helluva job of flying
(i think the a/c is a D model, looking at the props)

#3 re:above; notbut i but there a significant sphincter clenching event happening just about the time the crew saw the speedometer was not winding down and road in front was.
