#1 if it is 5000' below sea level the radiation near the sub could be 500 million times normal and it would be no big deal, e.g. gamma radiation loses about half its energy for every foot of water, neutrons can be a bit tougher but still get attenuated fairly quickly

#4 If the RX vessel is cracked then that could be fuel and byproducts being chuffed out. I imagine that there's a steam bubble forming until it reaches the point of the breach and that puffs out releasing the waste.
