#1 More likely taken from Syrian stockpiles, though other possibilities are former Russian Soviet stock from the 'stans, or the Balkans, or the handiwork of former Russian Soviet trained chemical warfare experts.

#4 Mustard gas (see blister agent above) is easy for pretty much anyone with a basic understanding of chemistry. Deploying it through artillery shells is complicated and requires a mechanical engineer. Not impossible for ISIS to make on their own, but I bet an outside force is supplying shells and/or knowledge on how to make them.
