#1 More likely taken from Syrian stockpiles, though other possibilities are former ~~Russian~~ Soviet stock from the 'stans, or the Balkans, or the handiwork of former ~~Russian~~ Soviet trained chemical warfare experts.

#4 Mustard gas (see blister agent above) is easy for pretty much anyone with a basic understanding of chemistry. Deploying it through artillery shells is complicated and requires a mechanical engineer. Not impossible for ISIS to make on their own, but I bet an outside force is supplying shells and/or knowledge on how to make them.
