#1 While it is common for LEO's to carry cocked and loaded in ready to fire position in the holster. Many with the safety on, but some with it off.

In digging up a few investigative test videos on how accidental discharges happen. Tests, while holstered lean to situations where the pistol is not fully, or properly holstered.
