#1 It was interesting note that in the '80s the Soviet army had no real NCO corps. Officers were doing what American NCOs did. So, I suppose these guys were doing things that battalion commanders would do in our forces. That or it may be Commissar kills, 'pour encourager les autres' which is also an old hold over from the Soviet army.

#2 Like that scene in "Enemy At The Gates" where Bob Hoskins invites an unsuccessful commander to "skip the paperwork"?

Or some combination of:
1- Senior officers going to the front because their comms are so fouled up that they're basically relying on word of mouth
2- Ukrainian penetration of of Russian "rear" areas.
3- The Ukrainians having very very good intel on who's where.
