#1 there are multiple and redundant collision avoidance systems on both the Navy ship (which was not a destroyer) and the container ship - somehow all failed or had been turned off or were ignored

also multiple accident alert systems - all of which failed or had been turned off

investigation reports will be brutal

The USS Fitzgerald is classed as a DDG - Guided Missile Destroyer.

As for the rest, it'll have to wait until the investigation is completed. And yes, it'll likely be brutal.

#3 A lot of speculation surrounding the track of the Filippino vessel and the discrepancies in the reported time of the collision.

Most major accidents result from an event cascade, which will probably be the case here.

#4 The guy over at GCaptain has set off a freakin firestorm over his editorial "The USS Fitzgerald Is At Fault. This Is Why."
Basically, both parties are responsible for collision unless one of them is tied up to a pier.
The commenting public is not interested in that part, however.
The analyst over at Jane's Ships said two days ago that the Crystal was evidently on autopilot based on the way it returned to course track after collision; no human pilot would do that (the likely reaction would some variation of WTF!)
